Ordering a Birth Certificate in Alberta

Need a birth certificate in Alberta? At Chaparral Registry & Insurance in West Lethbridge, we make the process quick and straightforward. Whether you're requesting your own certificate or applying on behalf of someone else, here's what you need to know before you visit our office.
To order a birth certificate, you must fall into one of the following categories:● Self (ordering your own certificate)● Parent● Legal guardian
Please bring:● Valid, government-issued photo ID
Legal Name Change Requirements:If your valid identification is not written in English, you must also provide an English translation accompanied by an affidavit from the translator stating the translation is accurate. To help shorten your wait time, please fill out the following form and bring it with you to the registry office.

Legal Name Change Services in Alberta

Need to change your legal name in Alberta? Chaparral Registry & Insurance in West Lethbridge offers in-person support to help you complete your Legal Change of Name (LCN) application accurately and efficiently. This process must be done at a licensed Alberta registry office, and we're here to guide you every step of the way.
To begin, visit our office to pick up the required LCN paperwork. We'll help you fill it out and ensure your documentation is in order. You must provide a valid, government-issued photo ID and complete all information before submission.
Who Can Apply for Legal Change of Name Documents?You may apply if you are:● The adult person whose name was changed● The individual who submitted the original name change application● A guardian or trustee of the person whose name was changed (must provide legal documentation)● A designated agent with completed consent and statutory declaration● A person holding power of attorney (must provide legal documentation)● A person with a court order from Alberta authorizing the application (must provide the order)
Changing Your Last Name After MarriageYou do not need to apply for a legal name change to take your spouse's last name. Bring your official marriage certificate and valid photo ID to our office—we'll help you with the update.

Alberta Death Certificate Applications & Eligibility

At Chaparral Registry & Insurance in West Lethbridge, we provide support for applying for Alberta death certificates and medical certificates of death. Whether you’re an executor, next-of-kin, or a legal representative, our team ensures the application process is clear and handled with care. Visit our office to access the necessary forms and receive assistance with your submission.
To apply for a death certificate, eligible individuals must provide identification and supporting documentation. Applications must be submitted in person at our registry location.
Who Can Apply for Alberta Death Certificates?Eligible applicants include:● The executor, personal representative, or estate administrator (must provide legal documentation)● An adult next-of-kin* of the deceased (must provide proof of relationship or Affidavit of Relationship)● A guardian or trustee for the deceased (must provide legal documentation)● A person with an Alberta court order (must provide a copy of the order)● A joint tenant of the deceased (must provide a certified property title)● A designated agent for an eligible person (must complete consent and statutory declaration)● An adult relative if no other eligible person is available (must provide proof of relationship or Affidavit of Relationship)● A funeral home representative (must provide proof of occupation)● A lawyer for an eligible person (must provide a valid Law Society card)● A public trustee needing documents per the Public Trustee Act
*Next-of-kin includes: mother, father, brother, sister, children, spouse, or adult interdependent partner (excludes in-laws, grandchildren, aunts/uncles, step-relatives, nieces/nephews).

Medical Certificate of Death: Certified CopyA Medical Certificate of Death is a certified copy of the original statement of death issued by a physician.
